Alabama Healthcare Hall of Fame Nomination Information

Criteria: A candidate to the Alabama Healthcare Hall of Fame should have by his or her work and accomplishments brought honor and fame to the field of healthcare. The candidate should have significant connections to the state of Alabama through his or her career or residency.

 The nominating committee carefully reviews the nominations from each field. The selected nominees are honored at an induction luncheon that is held at the Alabama Activity Center in Montgomery. The induction is held every other year and the deadline for nominations is October 15, 2005, 2007, 2009, etc.

 In addition to supportive materials, the following information is requested on each nominee:

  • Name of individual nominated for induction
  • Nominee's place of residence (or last place of residence if deceased).
  • Nominee's field or position of service in, or for, healthcare (medical practice, dental practice, nursing, academic medicine, healthcare administration, research, public health, volunteer, public service, business, etc.)
  • Nominee's distinctive service and/or contributions for healthcare in Alabama or elsewhere.
  • Individual or organization presenting this nomination.
  • Signature, date, address, telephone of nominator.
